Please note that the VHSL Membership Application has been redesigned with the following features:

     Enter the number "1" beside each sponsored activity.  Fees will be 
     calculated automatically.

     Enter the number of student athletic and cheerleader participants for 
     insurance and the fee will be calculated automatically.

     If you owe a penalty, please calculate the penalty amount manually and 
     then enter the amount on the form.  Totals will calculate automatically.

     Type your high school name, region and district where indicated.

You may complete the form on the computer but it MUST be printed out and manually submitted after it is signed by the principal, school board chairperson and superintendent.

Forms received via email will be deleted.  Please be sure to complete the form properly and mail the completed form and membership payment to VHSL by the deadline date!
